My 2 cents,
When things are bilateral I always look at the spine. Either he has some strange vibration being set up in the seat that is causing some inflamation or he is piching the nerves at the ishial tuberosities. Either way it is in the seat pan and you should look at dampening materials. Maybe even look at wheel chair devices that help spread the pressure in the seat pan. Good luck.
